Electronic Fund Transfer

CPHA uses direct deposits (also known as electronic fund transfers) for all payments. In order to facilitate your payment, please complete an authorization form and return it, along with a void cheque or a direct deposit slip, to ap@cpha.ca.

You can access a void cheque or direct deposit slip from your online banking platform. 

This information will be used only for the payment of monies owed to you and is stored on a secure server. This information can only be used to deposit money to your account, it cannot be used to withdraw money. 

Completing this form accurately and returning it promptly will help ensure you receive your payment as quickly as possible. 

A payment notification will be sent to the email address provided on the form. Look for a notification of payment email from Beanworks (<no-reply@sagepa.com>) with the subject “Canadian Public Health Association (Sage 300) has sent you an EFT Payment”. This email notification will include the number of the invoice being paid. If you do not receive an email, check your junk mailbox. The payment on your bank statement will be from “Cambridge Mercantile Corp”.
